have you used the phone for at least 40 days in the USA? if so you can contact T-Mobile to unlock it for you
wow, thanks for the quick response - I used the phone for under 40 days, (about 30 days in total) but still have a price plan and intend to switch to the £3.00 pay as you go tariff to maintain the number for my next visit (in about 3 months time). So technically I will have a pay as you go plan for over 40 days but won't physically have used the phone on the t-mobile network for 40 days, so not sure if that counts!
well they normally like to see some activity on their network for 40 days but since you're keeping your number with whatever plan and I believe the phone is already paid off, then you can give it a shot, call them and I believe they will unlock it for you

I have an idea but let me ask you this, were you able to make wifi calling with that phone when you were in the US?
yes, I used wifi for some international calls, or the data allowance for FaceTime as well as the network for local calls.
well I just checked that and supposedly you can, so my idea is: if you know how to use VPNs try to have a US ip address and make a couple of wifi calls or so before calling them requesting the unlock code, hopefully your line is still active, it will use your minutes though, so a couple of short calls won't hurt

download the open vpn app from the app store it's free, it will ask you to import a tunnel thing just create an account and choose the server, it's all free then import the file to the vpn app and you're all set, when you get connected just go the myipaddress site and make sure the ip is different than your wifi ip address

Sorry - did not understand the reason for VPN - I thought with WiFi calling one could just call the US numbers from abroad from a phone registered to T-Mobile in the US for free or using minutes without running up international charges.
