Have you tried going back to switch2t-mobile.com to check the status? It should tell you exactly what was wrong with the submission. On the document that you submitted previously, did it specifically designate an early termination fee for the line that you moved to T-Mobile?
I'm sure it's frustrating having to resubmit multiple times without knowing exactly what info we're looking for or having more info on why the reimbursement won't go through. At this point, I'd recommend working with T-Force over Facebook or Twitter. You're able to send them screenshots of the pages you've already submitted as well.
