You're going to have to call or use social media to fix this issue since the Tmobile reps don't have access to your account.
Interesting. The various plans I have been on over the years, prepaid and postpaid, have all had directory assistance included at no extra charge.

Correct directory assistance is not included free of charge. it's quite possible that an accidental dial may have happened because as far back as I can remember each carrier that I have had their own contacts such as support and directory assistance have been included on the phone and been added to my contacts accounts over time.
I would have everyone take a look at your contacts list and call logs and see if perhaps that number is in there.
