From what I'm seeing, you'll need the On Demand Data Passes and a 3G/4G data connection in order to use your device as a hotspot. In this case, you'd be charged for the data feature needed to use the service on your hotspot phone.

I suggest you use a VoIP app, such as Skype, Viber of Hangouts.
If your carrier name shows T-Mobile WiFi, this means you can call in USA for free, or receiving phone calls for free. This means you are on WiFI-calling.
But if you are not on WiFi-Calling, then yes, it's $0.25/min.
The advice is good, use a VoIP app.
I created a Googel VOice number, which is 100% free. And then use the Google Hangouts app to call thorugh the internet.
At 128k it works perfectly.
If you have an iPhone and can use Facetime (voice only, no video), at 128k it works perfectly too...
