Have you tried calling 611? This is a guess, but use the phone that you want to use for the change. Is ther a T-Mobile store close? And the last suggestion try T-Force.

few months ago i spoke to a agent on phone and i ask him to send me notification about bill on E mail and my phone also at 703-568-4003.
after few weeks this was stopped and i dont know what happens. please FIX this.
