Have you met all the requirements to the letter?
Meaning you're a T-Mobile customer, you've had service however many days, the phone is paid off, etc?

Which operating system are you using Android or iOS? Each each type has its own method of unlocking. The Android being the easiest through the app on the phone. IOS through iTunes but it can get tricky.

Please make sure you've read over this: Unlock your mobile wireless device and all requirements are met. And as snn555 mentioned, knowing what type of device you're wanting unlocked would be handy for us to know.

The device has been paid off. I called T-Mobile numerous times, spent hours on the phone. They submitted unlock requests 3 times after which I received
Thank you for taking the time to contact T-Mobile. After review of the account we have determined that the device is not in use with the corresponding mobile number. T-Mobile policies and procedures state that the device must be in use on the mobile number requesting the unlock code
I provided the IMEI number dozens of times. T-Mobile customer support representatives are useless so far. No one can or is willing to investigate the issue.

Tmobile's reply explains why your request got denied.
The phone has to be associated and meet the usage requirements with the phone number that requests
the unlock code.
Is the phone number yours that requested the unlock code or is it a friend who has a Tmobile account requesting
the unlock code for you. If it's the latter the phone would need to show usage on that line.

The handset has been in use with t-mobile since June. The phone number is mine. Once again, I bought it out from T-Mobile.

I would contact T-Force through messenger on Tmobile's facebook page and have them
check to see if the imei # of the phone is associated with your phone number and if not have
them fix this issue. This could be a glitch.

It turns out T-Mobile sold me a locked phone 3 months ago. I spent hours speaking with useless customer support representatives over Skype to Go, since I am traveling. They tried playing ping pong game by transferring my calls between prepaid to postpaid departments back and forth, their statements were inconsistent with each other, demonstrated absolute unprofessionalism and unwillingness to handle this case.
Over all ten (10!) unlock requests and a demand to get money back have been made to finally get the handset I paid money for unlocked.
T-Force and any other customer support representatives keep asking the same information over and over again. It feels like T-Mobile either doesn't keep track of it's records or it's is hard to find that information on their side.
I can share the documented chronicles of this unbelievable effort with those who are interested.
By the way, I have to use VPN service in order to access my T-Mobile account from abroad since T-Mobile blocks their website to some IPs:

Sure ,show me the unlocked phone Tmobile sold you.
The only unlocked phones Tmobile ever sold were the Nexus and Moto lines and they didn't
carry most of those.

I guess I wasn't clear enough. I used to lease the handset and bought it out 3 months ago. It has taken about 3 months to get it unlocked.
