1) My understanding is no. you don't have to repay T-Mobile for their reimbursement for the Pixel 2.
2) According to the document I cited above, yes.
3) It looks to me like it's $140+(4x$10) or $180, unless you want device protection (which I don't advise). On the T-Mobile ONE plans, T-Mobile absorbs the taxes & fees.
You have to have been making payments on any phone for 90 days. You have to pay Verizon for the Pixel 2. T-Mobile will reimburse you for the pay-off. They don't pay Verizon anything. (A lot of people are confused about this.) You might be able to just port the 3 iPhones over to a T-Mobile ONE plan, without the PLUS option and use GetOutOfTheRed for only the Pixel (not sure). Keep in mind that you probably need to do the porting near the end of your Verizon billing cycle. Verizon will probably not prorate months for port-outs. You don't want to pay a full month's bill for just a few days.

Hi Mike. Just got your message after coming back from my business trip. I will go to one of the T mobile stores in town to move my family phones to T- Mobile. Thank you very much for your follow up.
