This is a user support forum and the handful of T-Mobile employees who are on the site don't have account access so you will need to contact customer support. They will for security reasons require the account holder (or someone with account holder permissions) go thru a security verification process before unblocking the device to ensure you are the owner, it is for your protection of course.
