Contact the T-force team on FB or Twitter, so they can get case started with the Rebate center as they can direct you to what exactly they need also I'd contact Costco as well to see what they can do on their end.

Thanks for the replies guys, luckily I was able to contact the Rebate Team via the phone and talk to someone who looked specifically into my submission.
I was able to resubmit the request with the proper documentation, Copy of Receipt from Costco and Photo of the boxes with the IMEI clearly shown.
I also submitted a scanned copy of the Promotion worksheet that Costco gave me.
So far the submission has not be denied, it's been 2 business days, and still in Received Status.
I think it's good, but will check it from time to time to be sure.
