I don't believe that your mom has to give T-Mobile any information about you since she is responsible for the line. You will be able to use your phone number and an email addy of your own choosing to sign up for your own access to your individual account usage info. That is the only place you would be giving any info about yourself.
You would also might choose to set up a Google Account for data backup purposes. Google doesn't really require that much info about you either.
I wouldn't have any concerns if I were you. Enjoy your new phone.

Just wanted to reiterate what artart so kindly provided - since your mom is the Billing Responsible Party for the plan, we should only need her ID to verify her account.
