Check your advanced features. Also I don't have a problem with it automating when pulling from my face.

Maybe because you're right handed? Did a quick search and people who place the phone on the left side of the face somehow gets the ear or face to accidentally press the hold button.

I use my left ear when on the phone. I am left handed :)

I am going to say it only happens when someone calls you correct? I notice when you call out its on the bottom right of the phone and it can't be the ear or cheek that can touch it.
If someone calls there's a flag at the top which you can swipe away. Then the call would permanently be on the notification shade or you have to open the dialer.
Looking to see if there is a way to turn off that flag.
