I bought an unlocked E3276s-500 from AliExpress and can only get 2G/3G speeds (2Mbps down, 1Mps up). This device is supposed to support the T-Mobile LTE band 4 (AWS).
In the network scan (below), it shows A&TT and Verizon (311-480) LTE, but only 2G/3G for T-Mobile. I am using the SIM card from my iPhone SE that connects to the LTE network just fine.
Any ideas on why it doesn't work? Is there anything I can do to configure it? The USB modem is SIM-unlocked, with a Polish operator (Cyfrowy Polsat) logo on it; photo below.

My guess is you don't live in an LTE area covered by Band 4 or Band 2.
T-Mobile's coverage maps accentuate the Band 12 coverage.
I think you have to click up in the top right and say "My device isn't listed" or "I brought my device from another carrier" to exclude Band 12 in the maps?
If the cover is from a Polish network operator, i think the dongle should be E3276s-150, NOT S-500. Can you use DC-unlocked to detect it and see the result?

Have you changed the APN to fast.t-mobile.com?
There is also the possibility that the T-Mobile network does not recognize it as a USB data stick
T-Mobile does work with USB Modems.
I've been using the Huawei E397Bu-501, which has support for bands 12 and 4, for a year and a half on T-Mobile and haven't had connection issues. It almost always sees an LTE signal.
The model Huawei E3276s-505 has support for bands 2, 4, and 12, while the -501 only supports bands 2 & 4.
Can you confirm that the Huawei E397Bu-501 has Band 12 support that is compatible with T-Mobile?
So far, the specs that I have found indicate
4G LTE FDD - band 17( 700Mhz Lower B), band 4(1700/2100MHz)
which would not be T-Mobile's Band 12
I'm in a location that only has Band 12 connectivity
