Wow, that is certainly not a normal occurance. Can I suggest going to Menu> Settings> Applications> tap the All tab> locate the Mail app> select Clear data. *Note: This will clear any other email account settings already added in the app. Afterwards, restart the phone and retry. If the issue continues, there may be something happening within the app itself which can be corrected with a reset of the phone (see http://support.t-mobile.com/docs/DOC-2077).
Alternately, you can download a free app from the market such as K9 Mail and add your accounts through there.

Thanks very much for your assistance. I was able to resolve the issue, which turned out to be a case of operator error; instead of clicking on the Email app from within the "Set up your phone communication" dialog box (when adding my other gmail accounts,) I was going directly to the apps window and taping the Email app there, which only allows creation of a new account. Once I went through the "Set up your phone communication" wizard, I was able to add my other gmail accounts.
I did try all your suggestions and only discovered what was going on after I performed a master reset. I like the k9 mail app, but what I don't like, is the fact that any account I set up with an alternate email app doesn't show up in "accounts & sync."
Unfortunately, I can only access the other gmail accounts by going though the gmail app ui >menu >accounts and tap on the gmail account that I want to use. The method that I used on my old mt3g, was to add the other gmail accounts to the generic email app, which worked great, allowing me to keep my alternate gmail accounts seperate from my main account.
Additionally, I found that there is no way to associate a gmail account with the generic email app (which I was able to do on my mt3g); even when going through the "Set up your phone communication" dialog ui.. IF I select "other email account" and type in a google account," the ony option, is to create a "new account" which fails (as stated in my original post.) In fact, it is interesting to note, that in the "set up email" wizard, if one selects "other mail account," that you are presented with the "new account" dialog with no option to "add" account>sign in, like the gmail account wizard. I don't have a non-gmail account so I can't test it to see if the reation is different.
Ultimately, If I want to access my alternate gmail accounts from a separate widget/app, I'll have to install a different email app like k9 mail, since the generic email app appears to be useless. T-mobile/google didn't really think this one through....

When I attempt to add existing email accounts to the mail app, (not to be confused with the gmail app) it reacts as though I am attempting to create a new account in gmail. I get a pop-up that states: "warning the account name already exists." The procedure for this process is not covered in the documentation. I had no problem with this on my old mytouch 3g.

Well, after looking at this issue again; I found that once again, it was operator error.
the instructions from HTC's website:
How to add a POP3 or IMAP e-mail account close
- From the Home screen, tap the All programs button.
- Tap Mail.
- If an email account is already configured, first do the following:
- Press the Menu button.
- Tap Account list.
- Tap New account.
- Tap Other (POP3/IMAP).
- If necessary, tap Email address to display the keyboard.
- Enter your e-mail address.
- Tap Password.
- Enter your password.
- Tap Done to close the keyboard.
- Tap Next.
- "If desired, change the name of the account."
- Tap Finish setup.
Step 11. states "<if desired>, change the name of the account." Which is fine, but if you select gmail as the email type, the name of the account defaults to "GMAIL" which will then subsequently fail because the wizard will think that you are attempting to add the root gmail account. Once I changed the name to reflect the email address account name that I was adding, then it worked. Example: if the email address that I'm adding is firstname.lastname@example.org, I changed the name from GMAIL to "jdoe."
The directions in the mt4g slide user manual describes the instructions a bit differently:
3. Enter the email address and password for your email account and then tap
4. If the account type you want to set up is not in the internal database, you’ll be
asked to enter more details.
5. Enter the account name and your name and then tap Finish setup.
In both cases (HTC instructions and T-mobile mytouch instructions) need to be rewritten to explain the procedure when adding additional gmail accounts.
But wait, there's more...
I'm now running into another issue where it will not sync all my mail, the default setting is 3 days. If I change the download options for each additional email account to download all the mail in order for sync to download all the mail, put then I found that the mail app/sync will start downloading all the mail again, based on the sync schedule; thus, downloading additional copies of already downloaded mail. This wasn't the behavior the the mail app in my mt3g; in fact, the mt3g's mail app didn't have "download options." Now I'm back to either adding the additional gmail accounts to the GMAIL app or installing K-9 mail in order to keep them separated.
The question that I have for T-mobile and or google, is why change something that isn't broken and change the behavior of the mail app, that in my view rendors the app completely useless!
I'm glad that the email setup finally went through for you, and I appreciate you posting the steps here for others. Regarding the gmail app and any suggestions for it, have you tried searching for it in the market then selecting one of the contact methods under 'Developer info' to pass along your suggestions to Google? They do take feedback into consideration.
