I had this same issue on my Samsung (different model, similar software). Once you have unpaired the device, it will still show up on the phone, but if your click "Search for devices", as long as that device is not in range, it will disappear from the list.

I suppose if I never reboot my phone, and never turn bluetooth off and back on, it might not show back up. But, that is not a realistic way to use a phone. All smartphones need rebooted from time to time. And why waste battery power by leaving bluetooth on all the time? I turn it off when not used.
Any time bluetooth is turned off and back on, that device shows back up as a paired device...even though I had unpaired it dozens of times.
Any other thoughts?
The only other suggestion I have is to do a master reset. That will definitely remove it, along with all of your other information. I'm not sure if you are willing to lose everything for that, but if you are, that is the way to go.
